Your Name
Enter your first and last name i.e. John Smith. This field is required and must be completed.
Online Communication
Select a program that you use to communicate and send instant messages online. If the program you use is not listed leave this field blank, and then enter the name of the program you use before your username in the next field.
Username or Number
Enter the username or number for the instant messenger or online communications program you use. If you use a specific server enter it in the "comment" box below. Multiple names may be entered as well. This field must contain at least two characters i.e. 12
Email Address
Enter your email address into this field. This field must be completed with a properly formatted email address i.e. me@myself.com.
Your State
Enter the the abbreviations of the State wherein you live. This field must contain only 2 characters i.e. CA. Note that this field is also limited to 2 characters and will then auto-skip to the next field. This is accomplished by yet another Javascript (see source for auto-skip).
This field will also check if the abbreviation you typed is a valid state. (if you do not live in the US or Canada type 00)
Web Sites
Enter your URL here. If you don't have a website enter you favorite site or your favorite DHTML / JavaScript website. You may add a short description of this site in the next field - "comment". The address must contain "http://". This field is required and must be completed.
